UPDATE Nov 18, 2021: The Infusion Data (F2006) Reporting Overview eLearning has been transitioned to the Portal. The Pre-TED F2400 and Baseline F2000 eLearnings have been retired. Please reference the Forms Instruction Manuals for data reporting instructions.

Application Series

FormsNet3 Recipient Application Training (40 minutes)

FormsNet3 is the system we use to collect and store Recipient transplant data. The purpose of this training module is to familiarize the new user with FormsNet3 features and functionality. Access the eLearning here.

Query Functionality (25 minutes)

CIBMTR staff have the ability to place a query directly on certain questions in FormsNet3 which allows the data manager to see immediately where queries need to be resolved. This training will demonstrate through video how to respond to an active query, ultimately saving time in the resolution of data quality issues. Access the eLearning here.
