If you wish to reactivate your CIBMTR center membership, download the
CIBMTR Center Reactivation Form to complete and submit electronically. Make sure to use the Submit Form button to send electronically to the CIBMTR center membership team. If you have questions, email email@example.com.
request changes to your CIBMTR reporting track for HCT or cellular therapy, please
submit your request via our online questionnaire HERE. If your center's name, website, or another type of change needs to be made, please complete the online questionnaire HERE, downloading and completing associated forms as requested. A ticket will be created for you and you will receive a notification with the number in two business days or less. After completing the forms please attach them to your ticket.
If you wish to deactivate your CIBMTR center membership, center closure or withdrawal from reporting, please complete the closure or withdrawal questionnaire HERE. A ticket will be created for you in CIBMTR Center Support. If you have questions prior to completing the questionnaire, please submit a ticket via CIBMTR Center Support under CIBMTR Center Maintenance > Center Closure/Withdrawal from Participation.
