Please note that a permission request is subject to the following conditions:

• Appropriate credit for all CIBMTR data is required.
  
• Except as provided by CIBMTR, no adaptation or changes shall be made to the CIBMTR material without prior written consent of the CIBMTR.
  
• The CIBMTR material shall be used only in the manner as described in the Publication Permission Request Form. If CIBMTR material is sought for inclusion in a new edition or updated version to a document or publication from which copyright permission is currently being sought, a new CIBMTR Copyright Permission Request Form must be submitted to the CIBMTR.
