Use the form below to request permission to reproduce CIBMTR slides or data. Your request will receive prompt attention.
Please note that a permission request is subject to the following conditions:
- Appropriate credit for all CIBMTR data is required.
- Except as provided by CIBMTR, no adaptation or changes shall be made to the CIBMTR material without prior written consent of the CIBMTR.
- The CIBMTR material shall be used only in the manner as described in the Publication Permission Request Form. If CIBMTR material is sought for inclusion in a new edition or updated version to a document or publication from which copyright permission is currently being sought, a new CIBMTR Copyright Permission Request Form must be submitted to the CIBMTR.
